Underworld Actual (アンダーワールド実績 , Andāwārudo jisseki) is a hyper-advanced city that is deep below the normal world of Project Alicization with the only way to enter the city via a Rabbit Hole that is located in the outskirts of Centoria. No one aside from 5 turncoat integrity knights know of that specific rabbit hole when they turned on their brothers and sisters of the Axiom Church. The inhabitants are mostly made up of autonomous beings only known as card soldiers, who reside in their respective territories. There are 5 territories similar to that of Alicization, with those territories being the Kingdom of Spades, Empire of Clubs, House of Hearts, Republic of Diamonds, and the Castle of the Five Rulers. There is one ruler each in their respective territories (i.e. Queen of Hearts) and the ruler of Underworld Actual is known only as the Jack of All Trades, who and all the rulers of their respective territories reside in the Castle of the Five Rulers. As of currently, Hideo Matsuki/Requio is the current Jack of All Trades. All troopers are able to use enhance armaments to an extent. They are unable to use abilities since their own weapons themselves are the main power source for them. The factions also specialize in sects of combat. the Kingdom of Spades specializes in Swordsmanship, the Empire of Clubs specializes in Jousting, the House of Hearts specializes in Long-Ranged Combat, and the Republic of Diamonds specializes in Defense.

- This was the first Alicization project but all of the residents were not fluctlights, meaning most if not all of the residents were automatons, even the actual residents themselves.
- When Underworld Actual was found by the fluctlights, the residents of Underworld Actual were enslaved.
